The undersigned (“Member/Participant”) acknowledges and agrees to the following terms and conditions:

Voluntary Participation

I hereby voluntarily choose to participate in physical training, fitness, boxing, conditioning, and any other activities, whether inside the facilities of Vital Box Gym Inc., outdoors, or in any location arranged by the company.

Acknowledgment of Risks

I acknowledge that physical exercise and training involve inherent risks, including but not limited to: muscle strain, sprains, fractures, heart-related conditions, accidents, or aggravation of pre-existing medical conditions. I accept full responsibility for my health and safety while engaging in any activity offered by Vital Box Gym Inc.

Health and Pre-Existing Conditions

I certify that I am physically fit and have no medical condition that would prevent me from safely participating. I understand that it is my responsibility to consult with a physician prior to engaging in these activities. I release Vital Box Gym Inc. from any responsibility for conditions, injuries, or complications resulting from pre-existing health issues.

Waiver and Release of Liability

For and in consideration of being permitted to participate in any activity organized, sponsored, or operated by Vital Box Gym Inc., I, for myself, my heirs, executors, administrators, and assigns, hereby release, waive, discharge, and covenant not to sue Vital Box Inc., its directors, officers, employees, contractors, trainers, or agents, from any and all liability, claims, demands, actions, or causes of action arising out of or related to any loss, damage, injury, illness, or death that may be sustained, whether occurring inside the facilities, outside, or in any location where activities are conducted.

Indemnification

I agree to indemnify and hold harmless Vital Box Gym Inc. from any loss, liability, damage, or cost, including legal fees, that may result from my participation.

Governing Law

This agreement shall be governed by the laws of the Republic of Panama.
